Flexible work schedule is available to part-time and full-time positions that are compatible with telecommuting.
Benefits available after 90 days of employment for those working a minimum of 30 hours per week:
401(k) Savings Plan
Leader Dogs for the Blind’s 401(k) savings plan is available to all employees on the first of the month following their 90 days of employment. Leader Dog matches 5% at eligibility; employees are auto-enrolled at 5% and then auto-increased every year until they reach 10% on their 5th year of service; participants are fully vested after 3 years.
Medical insurance covers the employee, the employee’s spouse and eligible dependent children. Medical insurance includes extensive health coverage, prescription drug coverage, as well as mail order pharmacy services. Leader Dog pays 80% of premiums.
Dental insurance covers the employee, the employee’s spouse and eligible dependent children. Dental insurance highlights include diagnostic and preventive coverage at 100% for participating dentists and 50% for nonparticipating dentists. Leader Dog pays 80% of dental premiums.
Vision insurance covers the employee, the employee’s spouse and eligible dependent children. Vision coverage for in-network physicians includes eye exams, standard plastic lenses and contact lenses, once every 12 months (copays and allowances apply), frames once every 24 months. Leader Dog pays 80% of vision premiums.
Basic life and accidental death insurance premiums for policies valued at one year of the employee’s base salary up to $250,000 are paid for by Leader Dog.
Leader Dog offers all employees short term and long term disability insurance coverage paid for by Leader Dog. After a 14-day elimination period, the weekly benefit for short term disability is 60% of weekly earnings, not to exceed $1,000 per week; maximum period of benefits is 11 weeks. After a 90-day elimination period, the weekly benefit for long term disability is 60% of weekly earnings, not to exceed $5,000 per month.
Flexible Spending Accounts (FSA)
Leader Dog offers employees the opportunity to participate in a flexible spending account. Employees may set aside funds on a tax-free basis to a Medical Reimbursement account and/or a Dependent Care Reimbursement account for expenses that they anticipate to incur in the plan year. The Medical Reimbursement account may be used to set aside funds up to $2,500 per year for deductibles, co-payments or other services required for medical reasons but not covered by insurance. The Dependent Care Reimbursement account may be used to set aside funds up to $5,000 per year for the care of a dependent child.
